Contents of a Medical Report

1. History of injury, work, pre-existing medical conditions to the same area or of the same type, can refer to any attachements you provide them as such.

2. Physical examination including diagnostic testing and results of said tests.

3. Firm diagnosis and diagnostic codes must not use could have or may have, and if possible whether work caused or aggravated.

4. Causal Statement prefaced with "On a more reasonable than not basis the diagnosis of (condition) arises directly out of the injury or illness and/or aggravated by either acceleration or precipitation the medical conditions development and subsequent disability and then his supporting medical rationale and conclusion.

5. Prognosis, recommendations for treatment, periods of disability, limitations, duration of disability, permanence of condition, etc.

All medical reports/narratives should be addressed to the district director at the OWCP regional office you apply to. You should write a narrative to attach concerning the events if an injury and if an occupational disease a work history of all tasks you performed how often and how many days for how many years. You should ask the physician to pick up the original narrative yourself and mail it certified return receipt to OWCP after you make a copy or two for yourself if they lose it take a copy in and have an original signature put on the copy and then make another copy and mail it through your congressional offices.
